Amended 1 Any occupational therapist or occupational therapy assistant who plans to practice as a licensed occupational therapist or licensed occupational therapy assistant in the State of Georgia shall apply for licensure to the Board, on forms provided by the Board.
Rule Amended Three references will be required on forms provided by the Board from individuals who have knowledge of the applicant's professional training or experience within the last five years. This limited permit shall not be issued or renewed if the applicant has ever failed any exam s administered or approved by the Georgia Board of Occupational Therapy.
This permit shall allow the applicant to practice occupational therapy, for a period not to exceed 90 days, under the supervision of an occupational therapist who holds a current license in this state.
Amended 1 A license issued by the Board shall expire on March 31st of even numbered years. The license may be renewed upon submission of the renewal application and payment of the required fee, provided all requirements have been met. Refer to fee schedule. An application for reinstatement shall be required as provided for in Rule Practicing with an expired license is prohibited by law and practice during this period may result in disciplinary action for unlicensed practice.
Except as otherwise provided, each licensee is required is required to complete during each two 2 year renewal period a minimum of twenty-four 24 continuing education hours prior to the expiration date of the license. Failure to complete continuing education prior to expiration date can result in disciplinary action.
Acceptable professional continuing education activities shall include activities relevant to occupational therapy practice that can be deemed to update or enhance knowledge and skills required for competent performance beyond entry level occupational therapy. A cardiopulmonary resuscitation CPR course may not be submitted to satisfy any of the continuing education requirements or employer sponsored compliance coursework. This includes occupational therapy assessment, treatment planning, occupational therapy implementation and diagnostic related information.
This must include attendance at live presentations such as workshops, seminars, conferences, or formal academic coursework, or real-time, interactive webinars. To qualify as a real-time, interactive webinar, the licensee must have the ability to ask questions, receive an answer, and interact with other attendees at the time the session is taking place.
It may not be a pre-recorded session. These hours may be obtained online. These hours may also be related to administration, supervision, documentation, quality assurance and research. Each licensee randomly selected for a CE audit must submit to the Board supporting documentation as specified in this rule. Licensees and applicants for reinstatement may register earned CE credits for courses and experiences with an online recording and reporting system approved by the Board.
Licensees and applicants may register with a Board approved online system prior to April 1, to document CE credits for license renewal and reinstatement. A certificate of completion or similar documentation signed by program official, and 2. A shortened description may be accepted for programs specifically exempted by the Board such as AOTA and GOTA Conferences; b For "General" continuing education as referenced in this rule, if obtained at a conference, workshop or live presentation, required documentation is as noted above.
All other must include: 1. Explanation of the relationship of the activity to occupational therapy and your professional growth.
Date s and clock hours of the activity; 3. Other information as may be requested. A description of the fieldwork including name and type of facility, name of the fieldwork educator and times spent in direct supervision of the student; and, 2. Verification provided by the school to the fieldwork educator with the name of the student, school and dates of fieldwork or the signature page of the completed student evaluation form with evaluation scores and comments blocked out; d For published professional writing, applied research, and instructional presentations, as referenced in this rule, including first time or significantly revised presentations or an academic class session, workshop, seminar, in-service or professional meeting program session: 1.
Description of the presentation or research including location, title, date, hours of presentation, general content description, and type of audience; and, 2. Verification of the presentation, research or formal thank you note signed by the sponsor or program official. Verified instructional time by the course sponsor, a certificate of completion or similar documentation signed by the program official; and, 2.
Upon approval by the Board a limited permit will be issued. In ascertaining whether such an applicant meets such standards, the Board shall consider whether the school or course of training has been approved by the American Occupational Therapy Certification Board and whether the applicant's credentials have been evaluated and accepted by the American Occupational Therapy Certification Board.
If a duplicate pocket license is requested, the appropriate fee must accompany the request. The new supervisor will assure compliance with all terms and obligations outlined in this rule. The supervisor and alternate supervisor must hold a Georgia license in good standing under O. Title 43, Chapter 33, and have practiced full time for not less than one continuous year. The supervisor must regularly evaluate trainee performance in all areas as specified by the Board to include cosigning any documentation provided by the trainee.
At the end of the traineeship period, the supervisor must submit a performance evaluation on the board approved traineeship performance evaluation reporting form. The Primary supervisor must notify the Board of unsatisfactory performance at which time the training permit becomes null and void. The supervising therapist will supervise no more than two 2 trainees at one time. The supervisor must notify the Board within ten 10 business days when the trainee satisfactorily completes the traineeship.
Refer to fee schedule 2 Any applicant applying for licensure pursuant to O. The Board, however, may request further verification of any credential submitted if deemed necessary to evaluate the application. Refer to fee schedule 2 A military spouse or transitioning service member, as defined in O. Verification of an active license in good standing from another state board; and, 2. Within six 6 months of the issuance of a license, submit a verification of licensure in good standing from the state board of all states in which the applicant has held a license to actively practice; and, 3.
Verification of active continued competence in physical therapy in the two years immediately preceding the date of this application which may include a verification of employment or 26 hours of acceptable continuing education coursework pursuant to the requirements in Board Rule Official transcript from the institution granting the entry level degree in physical therapy or physical therapist assistant indicating the date of graduation; and, 5.
Passing scores from the national licensing examination. Passing scores from the Georgia State Board of Physical Therapy ethics and jurisprudence examination. Verification of an active license in good standing from another state; and, 2.
Within six 6 months of the issuance of a license, submit verification of licensure in good standing from the state board of all states in which the applicant has actively practiced in the two years immediately preceding the date of this application; and, 3. Official evaluation and transcript from a credential evaluation organization approved by the Board which shall meet the standards of the Federation of State Boards of Physical Therapy Coursework Tool CWT that was in effect at the time the applicant graduated from their physical therapy or physical therapist assistant educational program; and, 5.

All applicants who graduated from a non-CAPTE accredited school shall provide all of the above information. Verification of licensure in all states in which the licensure candidate holds a license or has ever held a license may be conducted by board staff.
All applicants who are graduates of educational programs conducted in a foreign country that are not accredited by CAPTE and approved by the Board must submit credentials prescribed by the Board in order to be considered for approval to take the licensing examination. Transcripts must be evaluated by a credential evaluation agency approved by the Board to determine if the professional instruction is substantially equivalent to that of entry-level United States-educated therapists.
Applicants educated in a non-English speaking physical therapy program must take and receive a passing score of 89 on the IBT TOEFL to include: a passing score of 22 on the writing section, a passing score of 22 on the reading section, a passing score of 21 on the listening section and a passing score of 24 on the speaking section before being allowed to sit for the licensure examination.
Applicants educated in an English speaking physical therapy program must have a school official submit an official letter to the Board attesting that the physical therapy curriculum was taught in English. Once a completed application is on file and prior to initiation of a traineeship, all foreign educated applicants must take and pass the NPTE and the examination on the laws governing the practice of physical therapy in Georgia and the rules of the Georgia State Board of Physical Therapy.
All foreign-educated applicants must complete a Board-approved traineeship consisting of a minimum of hours, which must be completed within three 3 months of the issuance of the traineeship permit. Applicants educated in a foreign physical therapy program that is accredited by CAPTE and approved by the Board, do not have to submit a credentials evaluation but they must meet the conditions of Board Rule Qualified applicants are: 1.
